A Case of Hepatocellular Carcinoma with Sarcomatous Changes

Abstract
Hepatocellular carcinoma with sarcomatous component has rarely been reported. The clinicopathologic characteristics; histogenesis; and prognosis after hepatic resection have yet to be clarified. We experienced a case of hepatocellular carcinoma with sarcomatous component. A 58- year-old woman had suffered from the liver cirrhosis due to chronic hepatitis B. She complained of dull abdominal discomfort in the right upper quadrant and general weakness. Abdominal computed tomography scan revealed a 4.2 x 3.8 cm sized mass with central low density and peripheral enhancement. The microscopic findings of the segementally resected liver mass showed hepatocellu- ar carcinoma with foci of pleomorphic spindle cell proliferation and transitional zone. The tumor cells revealed prominent nucleoli and frequent mitosis. The sarcomatous component reacted positivily for cytokeratin; vimentin and partially α-fetoprotein. Findings suggest that sarcomatous components rep?resent sarcomatous changes of hepatocellular carcinoma cells rather than the collision tumor of hepa?tocellular carcinoma and sarcoma.
