경증 코로나-19(COVID-19) 입원환자의 연령별 특성

Abstract

About 30~50% coronavirus disease 2019 (COVID-19) hospital inpatients is reported to have mild clinical presentation, and the clinical course is known to be good even in the elder population. In order to compare the clinical symptoms of mild patients with COVID-19 infection by age, the clinical symptoms and laboratory results were analyzed by dividing mild patients admitted to a dedicated hospital into three groups: under the age of 64, 65-84, and above 85 years of age. There was no difference in gender ratio by age among 555 mild cases. Concomitant diseases with significant differences by age were hypertension, diabetes, stroke, and coronary artery disease. Among the symptoms, cough and sore throat were different by age, and cough was significantly higher in the group under 64 years of age. The white blood cell count (WBC), neutrophil count, monocyte, blood sugar, procalcitonin, and creatinine were significantly higher in the 85 years old or older group, and the hemoglobin, albumin, and glomerular filtration rate (GFR) were significantly lower in the 85 years old or older group.

Introduction

2019년 12월 중국 후베이성 우한에서 발생한 이후 전 세계적으로 확산되고 있는 코로나-19(Coronavirus disease 2019, COVID-19) 감염은 고령자와 기저질환자에서 사망률이 높게 보고되고 있다[1]. 그러나 코로나-19 감염환자의 80% 정도는 경증이나 무증상이며, 경증환자는 고령자들도 젊은 연령과 동일한 임상경과를 가진다는 보고도 있다[2]. 경증환자의 연령별 임상증상을 비교하기 위해서 2020년 3월부터 6월사이에 코로나 전문병원으로 지정된 대구 동산병원에 입원한 코로나-19 감염환자 중 경증환자 555명을 대상으로 연령별 임상증상을 연구하여 앞으로 경증환자 진료에 도움이 되고자 하였다.

Material and Methods

이 연구는 계명대학교 동산병원 기관윤리위원회(2020-03-027호)의 승인을 받았으며, 2020년 2월 21일부터 4월 2일까지 대구 동산병원에서 코로나-19 감염으로 진단된 환자의 데이터를 분석하였다. 환자의 임상증상과 검사실 자료, 영상의학 결과를 후향적으로 수집하였다.
코로나-19 감염의 진단은 실시간 역전사 폴리머라제 연쇄 반응(RT-PCR)으로 COVID-19 RdRp, E, N 유전자를 확인하였다[3]. 경증과 중증의 구별은 산소 공급 또는 기계적 환기 지원 또는 추가 체외 산소화(ECMO)가 처방되었는지 여부로 결정하였다. 세계보건기구(WHO)의 중증도 기준(1. 호흡 곤란이 있거나 호흡수 ≥ 30회/분, 2. 산소 포화도 ≤ 93%, 3. PaO2/FiO2 ≤ 300 mmHg 또는 기계 환기가 필요한 경우)이 아닌 경우에[4] 경증 사례로 정의하였다. 위험도 연령 분류는 사망률의 차이를 보고하는 이전 연구[5]를 참고하여 64세 이하, 65~84세, 85세 이상의 3개 집단으로 분류하였다.
흉부 컴퓨터단층촬영(CT)은 영상의학과 전문의사의 판독에 따라 정상과 비정상으로 분류하였다. 병적인 폐실질의 이상 소견이나 지상 유리 불투명도(GGO)가 있는 경우에, 비정상으로 평가하였다. 입원 기간 중 반복 촬영한 경우에는 1번이라도 이상이 있으면 비정상으로 정의하였다.
통계분석은 범주형 변수는 빈도와 백분율로, 연속 변수는 평균, 표준 편차를 사용하여 분석하였다. 범주형 변수는 피셔의 테스트(fischer’s exact test)로 분석하였다. 데이터가 정규분포일 경우 연속 변수는 ANOVA test를 사용하고 그렇지 않으면 Mann-Whitney 테스트를 사용하였다. 통계 분석은 SPSS 버전 23.0 소프트웨어 (IBM Corp., Armonk, USA)를 사용하였다.

Results

일반적인 특성과 동반질환

총 경증 사례 수 555중 64세 이하는 406례, 65~84세는 142례, 85세 이상은 7례였다. 연령별 성비의 차이는 없었다. 연령별 유의한 차이가 있는 동반질환은 고혈압, 당뇨, 뇌졸중, 관상동맥질환이었다. 고혈압은 65~84세 집단에서 54/142명(50.5%), 당뇨는 26명 (24.3%), 뇌졸중은 8명(7.5%)으로 다른 집단보다 빈도가 높았고, 관상동맥질환은 85세 이상 집단에서 빈도가 높았다(Table 1).

증상

증상중 기침, 인후통이 위험 연령별 차이가 있었다. 기침은 64세 이하 집단에서 202/406명 (59.8%), 65~84세 집단에서 59/142명 (45.0%)으로 85세 이상 집단의 1/7명(16.7%)보다 유의하게 높았다. 인후통은 64세 이하 집단에서 98/406명(29.0%), 65~84세 집단에서 26/142명(19.8%)으로 85세 이상 집단보다 유의하게 높았다(Table 2).

검사실 결과

백혈구 수(WBC), 호중구 수, 단핵구 수, 혈당 농도, 혈중 프로칼시토닌 농도, 혈중 크레아티닌 농도는 85세 이상군에서 유의하게 높고, 혈색소, 알부민, 사구체여과율은 85세 이상군에서 유의하게 낮았다(Table 3).

Discussion

코로나-19 감염은 고령자와 기저질환을 가진 사람에서 사망률이 높다고 알려져있다[6]. 34세 이하의 사망률은 0.01%, 85세 이상은 27.1%에 이를 정도로 차이가 있으나, 생활치료센타에서 치료하는 경증환자는 연령에 상관없이 바이러스가 음전되어 치유되는 기간이 젊은 연령과 동일하다는 보고도 있다[2].
코로나-19 감염환자 전담병원으로 지정된 1개 종합병원에서 경증환자 555명을 64세 이하(n=406), 65~84세(n=142), 85세 이상(n=7)으로 나누어 비교한 결과 성비의 차이는 없었다.
연령별 유의한 차이가 있는 동반질환은 고혈압, 당뇨, 뇌졸중은 65~84세 집단에서 다른 집단보다 유의하게 높고, 관상동맥질환은 85세 이상 집단에서 빈도가 높았다. 심혈관계의 ACE-2 수용체를 통한 감염을 일으키는 코로나-19의 특성을 고려할 때 심혈관질환 위험요인인 고혈압, 당뇨, 뇌졸중, 관상동맥질환의 연령별 빈도의 차이와, 동일한 위험요인에 속하는 고혈압과 관상동맥질환의 빈도가 상이한 원인과 기전에 대해서는 추가 연구가 필요할 것이다.
증상중 기침, 인후통이 위험 연령에 따른 차이가 있었다. 기침은 64세 이하 집단에서 59.8%로, 65~84세 집단의 45.0%, 85세 이상 집단의 16.7%보다 유의하게 높아서 초고령 연령에서는 경증 일 경우 증상 표현 빈도가 더 낮았다. 인후통도 64세 이하 집단에서 29.0%, 65~84세 집단에서 19.8%로 초고령 집단보다 높았다. 개별 증상의 유무보다 증상의 개수가 중증도를 예측하는 위험 요소가 될 수 있다는 연구 결과와 함께 경증환자 예후 예측에 도움이 될 수 있을 것이다[7].
검사실 결과중 백혈구 수, 호중구 수, 단핵구 수, 혈당 농도, 혈중 프로칼시토닌 농도, 혈중 크레아티닌 농도는 85세 이상 초고령군에서 유의하게 높고, 혈색소, 알부민, 사구체여과율은 유의하게 낮은 결과도 경증환자 진료에 참고할 수 있는 결과이다. 혈색소, 알부민, 사구체여과율이 유의하게 높은 것은 나이에 따른 변화로 생각할 수 있으나 백혈구 수, 호중구 수, 구 수, 혈중 프로칼시토닌 농도가 높은 것은 고령자 코로나-19 경증감염에서 염증의 증가를 의미할 수 있으므로 추가로 확인하는 연구가 필요할 것이다.
연구의 제한점으로는 새롭게 발생한 코로나-19의 특성과 전담 병원의 특성 때문에 무증상 환자도 포함될 수 있다는 점과, 85세 이상의 경증환자 그룹의 수가 너무 적다는 점, 후향적 기록 고찰연구로 인과관계를 알 수 없다는 점이 있다. 그러나 무증상환자가 많은 생활치료센타에서의 경증환자를 대상으로 한 연구에[2] 비해서, 코로나-19 전담 종합병원에 입원한 환자들 중 경증환자의 임상증상과 검사실 결과를 고찰함으로서, 앞으로 계속될 코로나-19 감염환자의 진료에 참조할 수 있는 연령에 따른 임상 증상들을 고찰한 의미가 있다[8].
전담병원에 입원한 코로나-19 감염 경증환자를 64세 이하, 65~84세, 85세 이상의 3개 집단으로 나누어 임상증상과 검사실 결과를 분석하였다. 경증 사례수 555중 연령별 성비의 차이는 없었으며 연령별 유의한 차이가 있는 동반질환은 고혈압, 당뇨, 뇌졸중, 관상동맥질환 이었다. 백혈구 수, 호중구 수, 단핵구 수, 혈당 농도, 혈중 프로칼시토닌 농도, 혈중 크레아티닌 농도는 85세 이상군에서 유의하게 높고, 혈색소, 알부민, 사구체여과율은 85세 이상군에서 유의하게 낮았다.
