비심인성 흉통에서 고해상식도내압검사의 역할

Abstract

The most common cause of noncardiogenic chest pain is gastroesophageal reflux disease, and other esophageal motility disorders are also frequent. Esophageal manometry is important for diagnosing esophageal motility disease. Recently, high resolution manometry (HRM) has emerged and is reported to be more useful than conventional esophageal manometry in evaluating primary esophageal motility disease. In this study, the role and usefulness of HRM in noncardiac chest pain were investigated. From August 2018 to April 2021, 136 patients with chest pain were enrolled. Classification was carried out according to Chicago classification version 3.0, and 95 out of 136 patients showed normal findings, and 23 patients showed ineffective esophageal motility. When gastroesophageal reflux disease was classified according to Los Angeles (LA) classification, LA-M was 20% (6/30), LA-A was 63.3% (19/30), LA-B was 16.6% (5/30), and LA-C was, there were no patients with LA-D. Through this study, it was confirmed that about 22% of patients with noncardiogenic chest pain had reflux esophagitis, and about 30% of patients had esophageal motility disease. High-resolution esophageal manometry appears to be useful for the differentiation of noncardiogenic chest pain caused by esophageal motility disorders other than reflux esophagitis. After diagnosis of esophageal motor disease, follow-up studies on whether non-cardiac chest pain improves after drug treatment is required.

Introduction

특정한 심장질환이 없이도 반복되는 흉통을 호소하는 경우 비심인성 흉통(noncardiac chest pain, NCCP)이라 하는데[1,2], 일반인에서의 빈도가 약 30%에 이르는 것으로 알려져 있다[3,4]. NCCP를 일으키는 질환은 다양하며, 미세혈관 협심증, 근골격계 질환, 정신과적 질환, 종격동 및 늑막 질환, 결체조직 질환, 그리고 담도 및 위장관 질환 등 여러 원인이 관여하며[5,6], 최근 그 원인으로 식도질환이 중요시되고 있고 그 중 위식도역류질환(gastroesophageal reflux disease, GERD)이 가장 흔한 원인으로 알려져 있다[7,8]. 최근 연구결과에 따르면 한국에서 NCCP의 원인 중 위식도역류질환이 40%를 차지하였다[9].
GERD이외에 NCCP의 중요한 원인이 되는 식도 질환으로서 내장신경과민증, 식도운동질환 등이 비교적 빈번한 빈도를 차지한다[10]. 최근 식도질환의 중요성이 대두되고 있는데, 식도운동질환이 NCCP 환자의 30%를 차지한다는 서양의 보고도 있다. 식도운동질환의 진단을 위해서는 식도내압검사가 중요한데, 최근 고해상식도내압검사(high resolution manometry, HRM)이 등장하였고 식도이완불능증이나 미만성 식도경련 등의 원발성 식도운동질환을 평가하는데 있어 기존의 식도내압검사보다 유용함이 보고되고 있다[11,12]. 하지만, 아직까지 NCCP 진단에 있어 HRM의 유용성에 대한 연구는 많지 않다. 이에 본 연구에서는 NCCP에서 HRM의 역할 및 유용성에 대해서 알아보고자 하였다.

Materials and Methods

1. 대상

2018년 8월부터 2021년 4월까지 흉통을 주소로 계명대학교 동산병원 내과를 방문해 관상동맥조영전산화단층촬영(coronary computed tomography, CCT), 관상동맥조영술(coronary angiography, CAG), 경흉부심장초음파(transthoracic echocardiography, TTE) 검사에서 이상 소견 또는 악화 소견이 없어 NCCP에 대해 HRM과 위내시경검사를 시행한 139명의 환자를 대상으로 하였다. 자료가 누락된 3명을 제외하고 최종 136명을 대상으로 분석하였다(Fig. 1).

2. 방법

연구기간 동안 NCCP로 평가된 후 HRM을 시행한 환자들을 후향적으로 분석하였다. HRM 결과는 Chicago classification version 3.0에 따라 normal esophageal motility, achalasia, esophagogastric junction(EGJ) outflow obstruction, absent contractility, diffuse esophageal spasm, hypercontractile, ineffective esophageal motility, fragmented peristalsis로 분류하였다[13]. NCCP에 대한 추가 검사로 위내시경검사를 함께 시행하는 경우가 많아 이에 대한 결과도 함께 분류하였다. 역류성식도염의 중증도는 Los Angeles(LA) classification을 이용하였으며 점막 손상의 길이가 5 mm 이하인 경우 LA-A, 점막 손상의 길이가 5 mm 이상인 경우 LA-B, 점막 손상이 원주 방향으로 융합하지만 원주의 75% 미만인 경우 LA-C, 점막 손상이 원주의 75%이상에 걸쳐 융합하는 경우 LA-D로 분류하였다[14]. LA classification에서 역류성 식도염의 진단 기준이 되는 점막결손이 아닌 발적, 백색혼탁, 부종 등의 식도 점막변화가 있는 경우 미세변화 식도염 LA-M으로 분류하였다[14].

3. 통계분석

통계분석은 IBM SPSS Statistics ver. 20.0(IBM Co., Armonk, NY, USA)을 이용하였으며, 연속형 변수는 관측대상 수, 평균 ± 표준편차로 제시하고, 범주형 변수는 빈도와 백분율로 제시하였다. 통계적 유의 수준은 95% 이상으로 정하였다.

Results

1. 연구대상자의 일반적 특성

연령 분포는 24세부터 83세까지였으며 136명 중 남성은 70명, 여성은 66명이었으며 전체 평균 나이는 60.3세였다. 평균 체질량지수는 24.8 kg/m2이었으며, 남성에서 24.9, 여성에서 24.7로 성별에 따른 유의한 차이는 없었다. 현재 흡연자는 남성에서 22명, 여성에서 3명이었고 과거 흡연자는 남성에서만 25명이었다(Table 1). 동반 질환으로는 관상동맥질환이나 부정맥 등 심장관련 질환이 57건으로 가장 흔했고 당뇨와 고혈압이 각각 40건으로 뒤를 이었다.

2. HRM 결과

정상 소견을 보이는 경우가 136명 중 95명(69.8%)으로 가장 흔했으며, ineffective esophageal motility를 나타내는 경우가 23명(16.9%)으로 흔했다(Fig. 2). EGJ outflow obstruction이 9명(6.6%)이었고 hypercontractile은 3명(2.2%), achalasia, absent contractility는 각각 1명(0.7%)씩 있었다(Fig. 3). 평균 integrated relaxation pressure(IRP)는 10.6 ± 6.38 mmHg이었고 평균 distal contractile integral(DCI)는 1,603.3 ± 1,335.163 mmHg.s.cm이었다.

3. 역류성식도염 정도에 따른 비교

HRM을 시행한 136명의 환자 중 22명 이외의 114명(83.8%)의 환자는 위내시경검사를 같이 시행했다. 68명(59.6%)의 환자에서는 경도에서 중등도 위염 소견이 확인되어 가장 흔했고, 4명(28.6%)에서는 중등도 이상의 위염 소견이 확인되었다. 72명(63.2%)에서 위염이 확인되었다. 역류성식도염은 30명(26.3%)에서 나타났고 두 번째로 흔한 소견이었다. 그 다음으로는 위궤양, 십이지장궤양 소견을 보인 경우가 7명(6.1%)이었다. 그 외 열공탈장, 바렛 식도가 각각 1명(0.8%), 식도게실은 2명(1.6%)에서 확인되었고 식도염 소견이 1명(0.8%)에서 확인되었다. LA classification에 따라 역류성식도염을 분류했을 때 LA-M이 20%(6/30), LA-A는 63.3%(19/30), LA-B는 16.6%(5/30)였으며, LA-C, LA-D에 해당하는 환자는 없었다(Fig. 4).

Discussion

NCCP는 비교적 흔한 질환으로 삶의 질에 영향을 줄 수 있어 이에 대해 올바른 이해가 요구된다[15,16]. 이전 연구들에서 NCCP 환자에서 역류성식도염의 유병률은 10%에서 40% 이상까지 다양하게 알려져 있으며 본 연구에서는 약 22%의 유병률이 확인되었다. 이번 연구에서 역류성식도염 환자 중 의미있다고 판단되어지는 LA-A, LA-B에 해당하는 환자만 대상으로 한다면 NCCP 환자에서 역류성식도염의 유병률은 17.6%로 판단되며 이는 선행연구에 비해 낮은 수치이다. 이는 NCCP환자에서 역류성식도염 이외의 다른 원인에 대한 검사가 요구된다는 것을 시사하는 소견이라고 할 수 있겠다[17]. 최근 HRM이 등장하였고 achalasia나 diffuse esophageal spasm등 원발성 식도운동질환을 평가하는데 있어 기존의 식도내압검사보다 유용함이 보고되어 있으나[18] NCCP에 대한 활용도가 많이 보고되어 있지 않았다. 이 연구에서는 NCCP 환자에서 HRM 검사를 흉통 감별을 위한 검사로 활용하였고, 다양한 식도운동성질환을 감별해 낼 수 있었다. Ineffective esophageal motility가 가장 흔했으며 EGJ outflow obstruction이 뒤를 이었다.
이번 연구를 통해 NCCP를 호소하는 약 22% 환자에서 역류성식도염을 가지고 있었고, 약 30%의 환자에서 식도운동성질환을 가지고 있다는 것을 확인할 수 있었다. 연구의 제한점은 첫째, 환자의 수가 많지 않았다는 점과 둘째, 연구 대상 환자 중 이미 관상동맥 스텐트 시술을 받았거나 협심증 등의 심장질환을 가지고 있는 환자가 다수 포함되었다는 점이다. 이는 식도내압검사와 위내시경검사 전에 CAG나 CCT로 질병의 악화를 배제했으나 기존 질환 조절 정도를 알기는 어려웠다. 하지만, CCT, CAG, TTE 등의 방법으로 심장 흉통 환자를 체계적으로 배제한 것은 장점으로 생각되며, 등록된 모든 환자에 대한 데이터를 사용할 수는 없지만 상당수의 환자에서 내시경검사 결과를 분석하여 NCCP 환자에서 위식도역류질환의 유병률을 제안할 수 있는 점 역시 장점으로 생각된다.
결론적으로 역류성식도염 이외 식도운동질환에 의한 비심인성 흉통 감별을 위해 고해상도식도내압검사는 유용한 것으로 보인다. 식도운동질환으로 진단 이후 투약 치료 이후 비심인성 흉통 호전 여부에 대한 후속 연구가 필요하겠다.
